Recurrent Implantation Failure
Recurrent implantation failure (RIF) is a clinical condition where embryo implantation fails over multiple IVF/ICSI treatment cycles. It is typically defined by at least three previous embryo transfers of high-quality embryos without a confirmed pregnancy. This can be very demotivating and stressful for couples trying to get pregnant.
The RIF Clinic at Flinders Fertility, established by our Medical Director, Prof. Neil Johnson and Clinical Director Dr Toby Angstmann, is dedicated to the diagnosis and treatment of the causes of implantation failure. Our goal is to increase the chances of a successful pregnancy through a thorough assessment and individual treatment plan.
Investigating the Causes of RIF
RIF can be due to numerous factors, and we take a comprehensive approach to identify the causes. Some of the tests we can perform include;
1. Embryo Quality Assessment:
- Testing of embryos before transfer, including Preimplantation Genetic Testing for aneuploidy to confirm that the embryos have the right number of chromosomes.
- The assessment of embryo development in the culture medium to improve the conditions for embryo culture.
2. Uterine and Endometrial Evaluation
- 3D Ultrasound & Hysteroscopy to look for polyps, fibroids or adhesions in the uterus.
- Endometrial Receptivity Analysis (ERA) to determine the best time to transfer embryo.
- Endometrial Chronic Inflammation Testing to check for inflammation or infection that may prevent implantation.
3. Immune and Blood Clotting Factors
- Natural Killer (NK) Cell Testing to check the immune response that could be hindering implantation.
- Thrombophilia Screening to check for clotting disorders that may affect blood supply to the uterus.
4. Hormonal and Metabolic Assessments
- To exclude thyroid dysfunction, Thyroid Function Tests will be done.
- Insulin Resistance and PCOS Screening as metabolic disorders can affect implantation success.
5. Sperm Quality Investigations
- DNA Fragmentation Analysis to determine the health of the sperm and how it can affect embryo formation.
- Other semen tests which may help identify other male factor issues that may have been overlooked.
