Recurrent Implantation Failure

Recurrent Implantation Failure (RIF) is a condition where embryos fail to implant successfully in the uterus after multiple IVF cycles. Typically, RIF is diagnosed when high-quality embryos have been transferred at least three times without resulting in a confirmed pregnancy. This can be a frustrating and emotionally challenging experience for patients trying to conceive.

At Flinders Fertility, our dedicated RIF Clinic, led by Prof Neil Johnson and Dr Toby Angstmann, specialises in identifying and addressing the underlying causes of implantation failure. Our goal is to improve the chances of a successful pregnancy by offering comprehensive investigations and personalised treatment plans.

Investigating the Causes of RIF

RIF can result from a variety of factors, and our expert team employs a thorough approach to uncover potential reasons. Our investigations may include:

1. Embryo Quality Assessment

  • Advanced embryo testing, including Preimplantation Genetic Testing (PGT-A), to ensure the embryos have the correct number of chromosomes.
  • Assessment of embryo development in the lab to optimise embryo culture conditions.

2. Uterine and Endometrial Evaluation

  • 3D Ultrasound & Hysteroscopy to examine the uterus for abnormalities such as polyps, fibroids, or adhesions.
  • Endometrial Receptivity Analysis (ERA) to determine the optimal timing for embryo transfer.
  • Chronic Endometritis Testing, checking for inflammation or infections that may impair implantation.

3. Immune and Blood Clotting Factors

  • Natural Killer (NK) Cell Testing to evaluate immune responses that might be affecting implantation.
  • Thrombophilia Screening to detect clotting disorders that could interfere with blood flow to the uterus.

4. Hormonal and Metabolic Assessments

  • Thyroid Function Tests to rule out thyroid imbalances.
  • Insulin Resistance and PCOS Screening, as metabolic factors can influence implantation success.

5. Sperm Quality Investigations

  • DNA Fragmentation Testing to assess sperm integrity and its impact on embryo development.
  • Advanced semen analysis to identify any overlooked male factor issues.

Personalised Treatment Plans

After a thorough assessment, your fertility specialist will develop a tailored treatment strategy that may include:

  • Adjusting IVF protocols for improved embryo selection, immunotherapy or anti-inflammatory treatments to regulate immune responses.
  • Endometrial preparation techniques to optimise receptivity.
  • Lifestyle modifications and nutritional guidance.
  • Innovative techniques such as Embryo Glue

Frequently Asked Questions

RIF refers to the failure of embryos to implant, meaning a pregnancy never begins. Recurrent pregnancy loss occurs when pregnancies are achieved but do not progress beyond the early stages. Both conditions require different investigations and treatments.

Not necessarily. The first step is identifying the cause of RIF. In some cases, we may recommend treatments to improve implantation before attempting another cycle. If an IVF cycle is needed, we will optimise your treatment plan to increase your chances of success.

The timeframe depends on the number of tests required. Most patients complete the investigation within one to two months, allowing time to plan the next steps accordingly.

Yes, depending on the underlying cause. If the issue is related to hormonal imbalances, immune dysfunction, or uterine factors, targeted treatments may enhance fertility and improve the likelihood of both natural conception and IVF success.

Some tests and treatments may be eligible for Medicare rebates or private health insurance coverage. Our finance team can provide guidance on costs and rebates specific to your situation.
